Instructions
1. After submitting the form, please print two copies of the same.
2. One copy of the form along with the form fees, address proof (Aadhar Card or Voter ID or PAN Card)
of the parent, birth Certificate of the candidate from Municipal Corporation, Family photograph of the
child and ID Proof of the siblings if studying in Our school should be submitted at the school counter on
the specified date and time.
3. No form will be accepted on any other day except the specified date and time.
4. At the time of submission of form at the school counter both parents must accompany the candidate.
5. The selection committee reserves its right to select children for admission.
6. Since seats are limited there is no guarantee of admission to all applicants.
